Eligibility Requirements:

• Must have adequate income to meet household expenses
• Income must be at or below 200% of the poverty level

Must Provide The Following Information To Apply:

• Proof of income for the past 30 days
• Picture ID
• Social Security cards for everyone in the household
• Proof of address
• Written budget of household expenses
